Enclosed is an application for the V. R. Carter Memorial Scholarship
sponsored by the Oklahoma Foundation for the Education of Blind
Children and Youth, Inc. Please share this information with any
visually impaired students who will be graduating from high school
this spring and planning to attend a two/four year college/university
or vocational school. Feel free to make copies of the application as
necessary.

Scholarship Application

The Oklahoma Foundation for the Education of Blind Children and
Youth, Inc. is sponsoring the V. R. Carter Memorial Scholarship to be
presented to an outstanding visually impaired high school student in
recognition of extraordinary leadership, scholarship, enterprise and
service to others. The award of $1,000 will be given to a student who
is chosen by a selection committee of the Foundation.

The recipient will determine how the money will be used, e.g., tuition,
books, fees, equipment, etc. Applications will be accepted through
May 31, 2011. The recipient of the scholarship will be notified by June
15, 2011.

Eligibility Requirements:

To be eligible for consideration, applicant must:
1. Be categorized as visually impaired. Please send formal
documentation. (IEP or Eye Report)
2. Be a member of the current graduation class of a public
or private school in Oklahoma .
3. Have an overall grade point average of “B” or above,
based on grades 9 through12.
4. Be planning to continue their formal education beyond
high school, to secure training that will enable independence
and self-support.
5. Must be recommended by their school counselor or
principal.
6. Must have two additional letters of recommendation from
sources other than relatives.

Instructions:

Applicants who meet the eligibility criteria must submit the following
documents together in one package postmarked no later than May
31, 2011.
1. A PERSONAL APPLICATION AND STATEMENT
(attached which must be completed and signed by the
candidate.
2. AN ORIGINAL CURRENT TRANSCRIPT of the
candidate’s high school grades at the time of application,
covering grades 9 through 12.
3. A total of three letters of recommendation
a. School Counselor or Principal
b. Two letters of recommendation from persons other
than a relative
